29 CFR  1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  Worksite located at 117 Morgan Ave., Aurora, Missouri- On or about July 12, 2021, Employees were  exposed  to a shock and electrocution hazards in that the pumper truck's extendible arm came into proximity of overhead energized power lines causing the equipment to become energized and subsequently shocked and burned the employees in contact with the equipment.

29 CFR  1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  Worksite located at 117 Morgan Ave., Aurora, Missouri- On or about July 12, 2021, Employees were working and walking around the block foundation which had protruding reinforcing steel sticking out in which the steel was not guarded to protect from impalement hazards.
