29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  410 Buckminster Drive, Norwood, MA On or about, July 16, 2021, employees were exposed to a fall of 14.5-feet to the ground while performing gutter repairs and painting on a ladder jack scaffold platform without fall protection.   The middle sections of the 120-foot long ladder jack scaffold collapsed with at least two workers on it without fall protection, and striking one worker on the ground that resulted in the hospitalization of three employees.
