29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 07/16/2021, 11597 Tallfield Ln. - employees conducting roofing work from 5:12 pitched roofs of a two-story house were exposed to 19 foot fall hazards, in that, fall protection was not used.   b. On or about 07/16/2021, at 3824 Edgar Ave. - employees conducting roofing work from 5:12 pitched roofs of a two-story house were exposed to 19 foot and 9 feet 4 inch fall hazards, in that, fall protection was not used.  HUGH MACDONALD CONSTRUCTION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.28 (B)(15), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1350392, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 04/01/2019, WITH RESPECT TO A WORKPLACE LOCATED AT 15010 ABBY BIRCHNULL, TAMPA, FL 33601.
