29 CFR  1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:  Assembly Area; Flammable Cabinet: The following chemicals stored within the flammable storage cabinet were incompatible with the flammable chemicals stored within it:   a) Henkel, Bonderite M-CR 1200s AERO, (corrosive, oxidizer).  b) Sifco Process, Dichromate Conversion Coating 1720, (corrosive). c) Dubois Chemicals, Pentrate - Touch up, (corrosive).  d) Sodium Hydroxide Cleaner in a secondary container with a hand written label, (corrosive).

29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  Shipping and Receiving Area: On or about 19 July, 2021, fork extensions used for loading and unloading were used within the previous two weeks and the forklift data plate for the Hyster forklift, model # S60XM, serial # D187V05297U did not reflect the maximum capacity for the forklift with extensions attached.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical  Manufacturing Floor Area: On or about July 19, 2021, the employer did not ensure that each secondary container of hazardous chemicals in the workplace was labeled, tagged or marked with: pictograms, signal words, hazard statements, and precautionary statements.  a)  A spray bottle labeled as "Sodium Hydroxide" was observed in a flammable cabinet in the Assembly Area.  There was no indication of the hazards associated with the chemical. b) There were multiple instances of secondary spray bottle containers marked as acetone, soap, power clean, and alcohol throughout the facility. These containers were labeled using the previous hazard communication standard.
