29 CFR  1910.502(c)(2): The employer's COVID-19 plan was not in writing.  a) Doctors Subacute Healthcare LLC located at 59 Birch Street, Paterson, NJ:  On or about July 19, 2021, employees including, but not limited to Certified Nursing Assistants, Licensed Practical Nurses, and Registered Nurses provide direct care to suspected and confirmed COVID-19 positive residents and patients. The employer did not have a written COVID-19 plan.

29 CFR  1910.502(q)(2)(ii)(A): The COVID-19 log did not contain, the employee's name, one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced.     a) Doctors Subacute Healthcare LLC located at 59 Birch Street, Paterson, NJ: On or about July 19, 2021, the COVID-19 log, which recorded eight (8) employee cases, did not contain: one form of contact information, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced.
