29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Thompson and Thompson Construction of Ohio, on or about July 20, 2021, at the site location of 1236 Conneaut Ave. Bowling Green, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

Employees operating in this condition were exposed to eye injuries.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Thompson and Thompson Construction of Ohio, on or about July 20, 2021, at the site location of 1236 Conneaut Ave. Bowling Green, OH: The employer did not ensure the side rails of the aluminum extension ladder used on site to access the roof were extended at least three feet above the landing surface. One employee working from atop a residential structure roof was exposed to fall from height of 10 ft. 9" ft. while accessing work area from the ladder.
