29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  (a)TOJ Construction at 243 Curtis Street, Rapid City, SD 57701:  On or about and at times prior to July 20, 2021, the employer did not ensure that frequent and regular inspections were conducted of the job site.  Employees at this site were exposed to numerous fall and struck-by hazards.

This condition exposed employees to eye and face hazards associated with flying particles while utilizing pneumatic nail guns.

Employees were working on the roofs of the two adjacent properties near the edge with no fall protection system in place.  This condition exposed employees to a fall hazard of over six feet.

The condition exposed employees to the hazard of falling from an elevation of greater than 20 feet.
