29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).     On July 20, 2021, employees engaged in framing work over six feet without fall protection were exposed to fall hazards of approximately 11 feet to the ground below.

29 CFR  1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.     On July 20, 2021, the employer did not provide an extension ladder to employees to safely access and egress the residential building second level, exposing them to fall hazards.
