Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) 1307 Wheeling Ave (WV-2), Glen Dale, WV  - On or about 7/21/2021, an employee was not protected from falling when performing roofing work on a section of roof on the north side of the building, exposing the employee to falls of approximately 12-14 feet.

A ladder was not provided at all points of access exposing employees to falls of approximately 9 feet.
