29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) On or about June 28, 2021, for the employees who were exposed to electrocution hazards while setting flag poles near live electrical lines at or near 1981 HWY 82 in Mandaree, North Dakota.

29 CFR  1926.416(a)(3):When and where energized electrical power circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location, hazard involved and the proper protective measures to be taken:  (a)	On or about June 28, 2021, where employees were exposed to contact with overhead power lines while setting flag poles directly underneath live 14,400 volt electrical lines at or near 1981 HWY 82 in Mandaree, North Dakota.

29 CFR  1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  (a)	On or about June 28, 2021, where an employee was electrocuted and another received life threatening electrical burns and shocks  while installing a 30 foot flag pole which came in contact with a 28 foot high 14,400 volt power line at or near 1981 HWY 82 in Mandaree, North Dakota.
