(a) Location - 59 Willow Street, Hamilton, MA  On or about 06/14/2021, employees were exposed to crushing and struck by hazards, where the bidirectional Caterpillar 243B3 skid steer loader did not have an operable backup alarm which operated when the machine was moving in reverse.

29 CFR  1926.602(a)(9)(ii):Earth moving or compacting equipment which had an obstructed view to the rear was operated in reverse gear; such equipment did not have in operation a reverse signal alarm distinguishable from the surrounding noise level or did an employee signaled that it is safe to do so.  (a) Location - 59 Willow Street, Hamilton, MA  On or about 06/14/2021, employees were exposed to crushing and struck by hazards when earth moving equipment which had an obstructed view to the rear was used to reverse when this equipment did not have an operable reverse signal alarm (backup alarm) or an employee signaling that equipment could be operated safely in reverse.
