29 CFR 1926.102(a)(2):The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects. On or about July 23, 2021, a roofer was using a pneumatic nail gun to install shingles without wearing appropriate eye protection, exposing the employee to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about July 23, 2021, two roofers were installing shingles on a roof without fall protection, exposing the roofers to a fall hazard of approximately 25 feet to the ground below.

29 CFR 1926.1053(b)(1):29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about July 23, 2021, roofers accessed the roof of a home under new construction by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the roofers to a fall hazard of approximately 25 feet to the ground below.
