29 CFR  1915.158(b)(3):At least one 30-inch (0.76 m) U. S. Coast Guard approved ring life buoy with a line attached shall be located on each staging alongside of a floating vessel on which work is being performed  Two instances of inadequate life buoys were observed:     a. Dock 6 - The life buoy did not have a line attached to it; a 90' line is needed.  Employees work dockside and over water in this area and are therefore exposed to hypothermia and drowning hazards if they fall into the water. b. Dock 3 - The life buoy had a line that was less than 90' in length.  Employees work dockside and over water in this area and are therefore exposed to hypothermia and drowning hazards if they fall into the water.

Paint Shed, boatyard:  An employee required to don a 3M half-face, tight fitting respirator to protect against the health hazards associated with organic vapors generated when painting, using paint thinners, or grinding fiberglass, had a full beard preventing a proper face to mask seal.

29 CFR  1910.134(h)(1)(i):Respirators issued for the exclusive use of an employee shall be cleaned and disinfected as often as necessary to be maintained in a sanitary condition;  Paint Shed, boatyard:  An employee required to don a 3M half-face, tight fitting respirator to protect against the health hazards associated with organic vapors and dust generated when painting, using paint thinners, or grinding, was observed wearing an excessively dusty and dirty respirator.  The employee is exposed to skin irritation hazards.
