29 CFR  1926.501(b)(15):"Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system.  On or about July 26, 2021, during the installation of beam clips to Uni-span shoring scaffolding at a job site located at 211 Franklin Ave, Kent OH 44240; the employer did not ensure employees working up in the shoring scaffolding were protected from a fall of approximately 20 feet to the lower level. No fall protection system(s) was used.

29 CFR  1926.1051(a):A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On or about June 26, 2021, during the installation of beam clips to Uni-span shoring scaffolding at a job site located at 211 Franklin Ave, Kent OH 44240; the employer did not ensure portable ladders were available for employees to gain access and or egress to the working levels as employees were climbing the shoring scaffolding forms and the supporting braces.
