29 CFR 1910.134(e)(2)(ii):The medical evaluation did not obtain the information requested by the questionnaire in Sections 1 and 2, Part A of Appendix C of 29 CFR 1910.134: (a) At a jobsite located at 4722 Pear St, St Joseph, MO, employees were exposed to lead while performing tasks, such as, but not limited to, changing ventilation filters throughout the facility that manufactures lead containing batteries. Employees required to wear respirators at this location did not receive a medical evaluation that contains the information requested in the questionnaire in appendix C of 29 CFR 1910.134.
