29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  Location: back of building  On or about July 23, 2021, employees removing material scrap material from the ground were not wearing hard hats while scrap material above them was being thrown off the third floor.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about July 23, 2021, an employee was throwing scrap material out of a third floor balcony and was exposed to a height greater than 17 feet without fall protection.  b) On or about July 23, 2021, an employee was using a skill saw on a third floor balcony and was exposed to a fall greater than 17 feet without fall protection.  c) On or about July 23, 2021, an employee was working on the edge of a roof at a height of 25 feet without fall protection.
