29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Dj Maxx Contracting, LLC located at 4428 Grand Ave, Shadyside, OH 43947: On and preceding July 28, 2021,  the employer did not ensure that an adequate protective system was in place to prevent a trench collapse while performing work in a trench that varied from 7 to 9 feet in depth.  Work was taking place in a vertical wall trench which varied between 6 and 8 feet deep on a residential sewer repair without a means of cave-in protection in place. The following condition exposed employees to struck-by/caught-in hazards.
