29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Jobsite, 581 Philadelphia Street, Indiana, PA 15701:  On or about August 2, 2021, employees engaged in roofing work on the roof of a building with a slope of approximately 6 in 12 (vertical to horizontal) were not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  The employees were exposed to a fall hazard of approximately 25 feet below to the ground below.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) Jobsite, 581 Philadelphia Street, Indiana, PA 15701:  On or about August 2, 2021, the employer did not maintain a written certification record of fall protection training for each employee engaged in roofing work on the roof of a building with a slope of approximately 6 in 12 (vertical to horizontal).
