On or about August 4, 2021, at the jobsite, the employer did not ensure the operator of the powered industrial truck was competent to safely operate the equipment as demonstrated by the successful completion of training and evaluation of operation. On or about August 4, 2021, at the jobsite, the employer did not ensure the operator of a powder-actuated tool was trained, and certified by an authorized instructor, in the use and hazards of operating a powder-actuated tool. This exposed employees to struck-by hazards.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points. On or about August 4, 2021, at the job site, the employer did not ensure that an electrical extension cord was protected from damage including, but not limited to the following: a. The cord was placed on the ground and was driven over by a forklift truck. b. The cord was placed on top of sharp edges of metal studs without protection from damage. This exposed the employees to electrical hazards.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied. On or about August 4, 2021, at the jobsite, the employer did not ensure the scaffold platform was secured to the forks when an employee was working from the scaffold platform. This exposed the employee to a fall hazard.
