29 CFR  1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly   On or about August 2, 2021 at this workplace, employees operated a powered industrial forklift truck from Line #2 to unload polymer pellets into a roll dumpster and the portable fire extinguisher mounted on the forklift for Line # 2 was not subject to monthly visual   inspections.   Employees were exposed to fire hazards.
