29 CFR  1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  a) On or about August 6, 2021, at a commercial construction site, where an employee used the guardrails of a Haulotte, Optimum 1931E, aerial work platform to increase his working height, exposing him to a fall hazard.

29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) On or about August 6, 2021, at a commercial construction site, employees were exposed to a fall hazard while installing siding from a JLG Telescopic Boom Lift, Model 400S, without a fall protection system.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about August 6, 2021, at a commercial construction site, where an employee was exposed to a fall hazard greater than 6 feet when installing a canopy without a fall protection system.
