29 CFR  1926.1053(a)(1)(ii):Non-self-supporting portable ladder(s) were not capable of supporting without failure, at least four times the maximum intended load:  Jobsite (Ground Floor): The company did not ensure that the job made ladder was constructed and tested in conformance with the applicable provisions of appendix A, therefore exposing its own employees  to load refusal/ failure when accessing the second floor.
