29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a)  210 Back Fork Street, Webster Springs, WV:  On August 9, 2021, employees were exposed to a fall hazard of approximately 11 feet (eave to ground height) to the ground below while conducting roofing activities on the roof of a commercial building with a slope of 6:12.   D C Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1264692, citation number 1, item number 1a and was affirmed as a final order on October 24, 2017, with respect to a workplace located at 1565 Sugar Creek Road, Webster Springs, WV.
