29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident:  (a) Doral Automotive Dealership located at 11020 Southwest 186th Street, Cutler Bay, FL 33157:  On or about 8/10/21, an employee died of COVID-19. The employer did not report the work-related fatality to OSHA within 8-hours. The employer reported the death to OSHA on 8/11/2021.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   (a) Doral Automotive Dealership located at 11020 Southwest 186th Street, Cutler Bay, FL 33157:  On or about 8/12/21,  the employer did not provide effective information and training to an employee assigned to use chemicals such as, but not limited to, Surface Sanitizer to disinfect work areas.
