On or about 8/12/2021, at the new 7-11 construction site located at 1245 S Volusia Ave, Orange City, FL, an employee was exposed to a fall hazard of 132 inches to ground level, when working from an overhead canopy and not using a means of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. On or about 8/12/2021, at a new 7-11 construction site located at 1245 S Volusia Ave, Orange City, FL, an employee was exposed to a fall hazard of 132 inches to ground level, when using a ladder that was not extended 3 feet above a landing surface and not secured to gain access to an overhead canopy.
