29 CFR  1910.178(m)(1):Trucks shall not be driven up to anyone standing in front of a bench or other fixed object.  a) On or about August 3, 2021, the Toyota Forklift Model 8FGCU32 was used to move a steel tank.  During the move it was driven up to an employee standing in front of a fixed wall exposing the employee to struck by and crushing hazards.  No abatement certification or documentation is required for this item

29 CFR  1910.178(o)(1):Load(s) on powered industrial truck(s) were not stable or safely arranged:  a) On or about August 3, 2021, a 660 gallon steel tank, measuring approximately 10.5' high by approximately 3' in diameter and weighing approximately 1391 lbs, was moved using a powered industrial truck and was not stable causing it to shift and injure and employee.   No abatement certification or documentation is required for this item
