29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  a) On or about August 16, 2021, at the previously addressed job site,  employees accessed a roof (landing surface) via a Keller extension ladder and the ladder side rails that did not extend at least 3 feet above the landing surface.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   a) On or about August 16, 2021, at the previously addressed job site, employees tearing off a roof in preparation to reroof were not protected from falls to the lower levels.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).  E & P Exteriors Inc. dba Eber Portillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1446556, citation number 1, item number 2 and was affirmed as a final order on January 7, 2020, with respect to a workplace located at 516 Ranch Rd., Wheaton, IL 60187.
