29 CFR  1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a) On August 6, 2021; two employees installing roof shingles on a one story 6:12 pitch residential roof wore body harnesses that were attached to lanyards with excess slack.  Lanyards were attached to rope in excess of 18 feet in length. Eave to ground was measured at 8.7 feet, and roofers were working within 3 feet of the roof edge.   Note* Abatement certification IS required for this item.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   a) On August 6, 2021; two employees installing roof shingles on a one story 6:12 pitch residential roof wore harnesses that were attached to lanyards with excess slack.  Lanyards positioned at the lower back were attached to rope in excess of 18 feet in length. Eave to ground was measured at 8.7 feet, and roofers were working within 3 feet of the roof edge.
