29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing activities.  This was most recently observed on August 16, 2021, at 13403 S 35th St, Bellevue, Nebraska where two employees were working on a residential structure that had a 14 foot eave height and they were not using fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders to access elevated surfaces.  This was most recently observed on August 16, 2021, at 13403 S 35th St, Bellevue, Nebraska where employees were using a ladder to access the roof of a residential building and the ladder's side rails did not extend three feet past the eave of the roof.
