29 CFR 1926.501(b)(13):  The employer did not ensure that each employee engaged in residential roofing activities with unprotected sides and edges (6) feet or more above lower levels, was protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   Jobsite: The employer did not ensure that the employee who was working from the roof of the residential building under renovation was protected from fall hazards above six feet by the use of guardrails, safety nets or personal fall arrest systems.  On or about August 13, 2021, an employee sustained serious injuries from a fall from at a height above six feet to the ground below.  The employer routinely fails to assure effective protection against  fall hazard as required by the standard, as demonstrated on or about August 13, 2021, when employees were not protected from fall hazards where fall protection was not worn at the site in front of 660 Williams Street, Bridgeport, CT 06601.  To abate this violation, the employer must ensure that affected employees use appropriate fall protection when exposed to fall hazards while working above six feet from a lower level.    Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance; including an explanation of how these steps protect employees from falling from elevations above six feet that may result in injuries and possible death.

29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee was using appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  Jobsite:  The employees who were required to use pneumatic nail guns to install roofing shingles, were not equipped with and required to wear eye protection.    The employer routinely fails to assure effective protection against eye/face injuries as required by the standard, as demonstrated on or about August 13, 2021, when employees were not protected from struck-by/flying object hazards where eye/face protection was not worn at the site in front of 660 William Street, Bridgeport, CT 06601.  Guallpa Construction LLC was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a) (1), which was contained in OSHA inspection number 1094126, citation number 02, item number 001 and was affirmed as a final order on April 10, 2017, with respect to a workplace located at 301 Roosevelt Drive, Derby, CT 06418.  To abate this violation, the employer must ensure that affected employees use appropriate eye or face protection when exposed to eye or face hazards from flying particles   Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance; including an explanation of how these steps protect employees from eye or face injuries.

Jobsite:  The employer did not ensure that each employee required to work on a roof at a height of 6 feet or above had been provided with fall hazard training.    On or about August 13, 2021,  an employee sustained serious injuries from a fall above six feet to the ground below.  The employer routinely fails to assure effective protection against  fall hazard as required by the standard, as demonstrated on or about August 13, 2021, when employees were not protected from fall hazards where fall protection was not utilized and fall protection training was not conducted at the site in front of 660 Williams Street, Bridgeport, CT 06601.  Guallpa Construction LLC was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a) (1), which was contained in OSHA inspection number 1094126, citation number 2, item number 4 and was affirmed as a final order on April 10, 2017, with respect to a workplace located at 301 Roosevelt Drive, Derby, CT 06418.  Guallpa Construction LLC was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a) (1), which was contained in OSHA inspection number 1271567, citation number 1, item number 3a and was affirmed as a final order on February 15, 2018, with respect to a workplace located at 51 Diane Terrace, Waterbury, CT 06705.  To abate this violation, the employer must ensure that affected employees use appropriate fall protection when exposed to fall hazards and are provided with fall protection training while working above six feet from a lower level.    Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance; including an explanation of how these steps protect employees from falling from elevations above six feet that may result in injuries and possible death.
