29 CFR  1910.178(l): This is applicable to construction employment as referenced by 1926.602(d). The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer is failing to protect employee(s) engaged in powered industrial truck operations from crushing and struck-by hazards. This was most recently documented on August 18th, 2021 in Gretna, Nebraska. The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  The employer is failing to protect employees from fall hazards associated with roofing and framing operations. The most recent occurrence of this was evident during the inspection of the job site located at  10017 South 187th, Gretna, Nebraska 68028 - The employee observed working on the roof was not utilizing any form of fall protection - The exposed employee was working at heights greater than 6 feet.
