29 CFR  1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):  a) On or about July 12, 2021 the employer failed to ensure that effective machine guarding was in place to protect the employee from the point of operation on the part revolution power press (P-37 Heim 50 ton). Barrier guarding in place on the machine contained an opening to large allowing employees to reach the point of operation. The opening was not in accordance with table O-10 of the standard being that the opening was 3-4 inches well past the allowable maximum width to the hazard.   b) On or about August 17, 2021 the employer failed to ensure that effective machine guarding was in place to protect the employee from the point of operation on the part revolution power press (#P-5 Heim 50 ton). Barrier guarding in place on the machine contained an opening to large allowing employees to reach the point of operation. The opening was not in accordance with table O-10 of the standard being that the opening was 1 7/8" inches  in width and the distance to hazard was 7 1/4".
