29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).   On or about August 9, 2021, at the Reeves County Hospital Replacement, an extension ladder was not set at the proper angle (4:1 ratio) when leaned against the wall. This condition exposed employees to fall hazards.

29 CFR  1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall.   On or about August 9, 2021, at the Reeves County Hospital Replacement, an employee was observed carrying a drill in his hand while climbing up an extension ladder. This condition exposed the employee to fall hazards.
