29 CFR  1910.134(f)(1):The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT):  a)Serenity Hospice located at 56 Georgetown Road Bordentown, NJ 08505:  On or about 8/9/2021,an employee was using an N95 respirator 3M-8210, without passing an appropriate Qualitative Fit Test (QLFT). Instead, the employee had only been given an inadequate fit test that did not comply with QLFT protocol in that he was fit tested while wearing a beard. The employer required the employee to wear a N95 filtering facepiece respirator to protect against the SARS-CoV-2 virus while providing care to suspected COVID-19 residents.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION AND DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
