29 CFR  1904.7(b)(3):The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.   On or about August 25, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2021.  On July 30, 2021 an employee suffered a work-related injury requiring hospitalization and surgery, resulting in days away from work.  The employer classified the injury by placing a check mark in column J "Other recordable cases."

29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   On or about July 30, 2021 an employee suffered a work-related injury, resulting in hospitalization and the employer did not report the hospitalization to OSHA with 24 hours of  the incident.
