29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a) On or about August 24, 2021 at the jobsite employees  preparing the structure for plastering without head protection were exposed to struck-by hazards while working one above the other.

29 CFR  1926.452(c)(2):Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  (a) On or about June 24, 2021 at the site employees were exposed to fall hazards while working from the scaffold which was not braced for lateral security on one side of the scaffold.
