(a) Location - 51-53 Summer Avenue, Haverhill, MA  On or about 09/01/2021, employees were exposed to fall hazard while working on the pumpjack scaffolding with no mid-rails and no end-rails.

(a) Location - 51-53 Summer Avenue, Haverhill, MA  On or about 09/01/2021, employees were working on a pump jack scaffolding that only had one set of bracing installed.
