29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about August 31, 2021, at the residential construction site located at 20914 Velvet Wing in Cypress TX, employees were exposed to eye injuries while operating nail guns without wearing eye protection.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   On or about August 31, 2021, at the residential construction site located at 20914 Velvet Wing in Cypress, TX, employees were exposed to a fall hazard of more than 6 feet while doing framing work on the roof of a house without the use of any fall protection system.   Roberto Constante-Zabala was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.501(b)(13), which was contained in OSHA inspection number 1516772, citation number 1, item number 2 and was affirmed as a final order on 04-26-2021, with respect to a workplace located at 15010 Fischer Reservoir,�CYPRESS,�TX�77433.
