29 CFR 1904.29(b)(1):What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 1, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Pain". There was no description of what caused the employee's injury. b. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 5, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Puncture". There was no description of what caused the employee's injury. c. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 6, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Strain/Sprain/Muscle Ache". There was no description of what caused the employee's injury. d. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 22, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Abrasion". There was no description of what caused the employee's injury. e. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 22, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "<N/S>". There was no description of what caused the employee's injury. f. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On July 23, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Exposure - Body Fluids". There was no description of what caused the employee's injury. g. On or about September 2, 2021, the employer did not provide the required injury or illness description on the OSHA Form 300 for the calendar year 2021. Section F of the OSHA Form 300 requires a description of the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill. On August 20, 2021 an employee was injured and Section F of OSHA's Form 300 listed the injury as "Strain/Sprain/Muscle Ache". There was no description of what caused the employee's injury.

29 CFR 1910.502(c)(3): The employer must designate one or more workplace COVID-19 safety coordinators to implement and monitor the COVID-19 plan developed under this section. The COVID-19 safety coordinator(s) must be knowledgeable in infection control principles and practices as they apply to the workplace and employee job operations. The identity of the safety coordinator(s) must be documented in any written COVID-19 plan. The safety coordinator(s) must have the authority to ensure compliance with all aspects of the COVID-19 plan. On or about September 2, 2021, at the HSHS St. Mary's Hospital Medical Center; in all areas of the facility, the employer failed to document the identity of the safety coordinator(s) in the written COVID-19 policy.
