29 CFR  1926.453(b)(2)(v):A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At the jobsite located at 10850 N Summit in Kansas City, Missouri:  Employees were exposed to a fall hazard.  Employees were observed working in an aerial lift and not wearing a full-body harness or belt with lanyards attached to the boom or basket of the lift.  Employees were exposed to a fall distance of over 15 feet.

Employees were observed installing materials and working at height.  Employees were exposed to a fall distance of greater than 11 feet platform to ground without use of fall protection.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  At the job site located at 10850 N Summit in Kansas City, Missouri:  Employee was exposed to a fall hazard.   An employee engaged in painting  an exterior wall of the building was exposed to a fall hazard of up to 11 feet.  The ladder was located on a platform that was unguarded 11 feet above the ground, the step ladder was leaned against the building and not used as it was designed.
