(a) Production; Iron 1-3: On or about 8/26/2021 and at times prior, the employer exposed employees to slip hazards in that a broken valve on the water heater caused a spill on the floor.  Employees working in this area are exposed when they retrieve tools and other material.

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate  a) Production Area: On or about 8/23/2021 and times prior the employer exposed a maintenance employee to electrical and mechanical hazards in that training on the type and magnitude of energy sources in the workplace was not performed. The maintenance employee performs maintenance and service on equipment such as but not limited to the Miglus Control and the washers.

29 CFR  1910.1200(h)(3)(iv):  The details of the hazard communication program developed by the employer, did not include an explanation of the  safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) Facility: On or about 8/23/2021 and at time prior, the employees were exposed to chemical hazards, in that training on the safety data sheet was not provided to employees who were required to use hazardous chemicals such as but not limited to Turbo Fresh and oils used to lubricate machines.
