29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite located at 3402 J Street Philadelphia, PA- The employer did not provide training that allowed employees to recognize, evaluate and mitigate hazards associated with high voltage electricity.  Condition was noted to exist on or about August 27, 2021.

29 CFR  1926.416(a)(3):Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit:   a) Jobsite located at 3402 J Street Philadelphia, PA- The employer did not ensure that the employee utilizing the scissor lift did not come into contact with a 13.2kV live electrical line, thus exposing employees to electrical shock hazards.   Documented to occur on or about August 27, 2021.

29 CFR  1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) Jobsite located at 3402 J Street Philadelphia, PA- The employer did not ensure that the employee was protected against electrical shock from a live 13.2kV power line by effectively grounding or deenergizing the line before work was performed.  Condition was noted on or August 27, 2021.
