29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a)The Overlook Lot 196-198 Falling Waters WV: On or about September 8, 2021 and times priors, employees were working installing framing on a row of residential homes. Lumber debris with protruding nails was scattered throughout the site exposing employees to trip and puncture/impalement hazards.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) The Overlook Lot 196-198 Falling Waters Front Side: On or about September 8, 2021 and times prior, an employee was exposed to a fall of 10 feet frame to ground, while nailing in framing doing framing work on the front of a new residential townhome. The employee was not utilizing a fall protection system or fall arrest harness. b) The Overlook Lot 196-198 Falling Waters Right Side: On or about September 8, 2021 and times prior, an employee was exposed to a fall of 12 feet frame to ground, while nailing in framing doing framing work on the front of a new residential townhome. The employee was not utilizing a fall protection system or fall arrest harness.
