29 CFR  1926.20(b)(2):  The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) On or about September 16, 2021 at the Bull Run Power Plant "Charlie" and "Delta" precipitator insulation project, employees were exposed to fall, laceration, and electrical shock, burn, and electrocution hazards and the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person.

29 CFR  1926.1051(a):  Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  a) On or before September 16, 2021 at the Bull Run Power Plant "Charlie" and "Delta" precipitator insulation project employees were exposed to fall hazards when accessing the top of the electrical insulator compartments at a height above the roof of 61 inches to perform work while on the compartment and the employer did not ensure a stairway or ladder was provided to all personnel points of access where there was a break in elevation of 19 inches or more.

29 CFR  1926.501(a)(2):  The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely.  Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:  a) On or about September 16, 2021 at the Bull Run Power Plant "Charlie" and "Delta" precipitator insulation project employees were exposed to fall, laceration, electrical shock, electrical burn, and electrocution hazards while on double and quadruple electrical insulator compartments and the employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely.  Employees were allowed to walk and work on insulator compartments which did not have the requisite strength and structural integrity.
