Aurora, CO 80014: On or before 9/23/21 Supreme Service LLC as the creating and exposing employer did not protect employees from injury in that roofing employees were not wearing fall protection as required by this standard. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Supreme Service LLC at 4189 S Fraser Ct. Aurora, CO 80014: On or before 9/23/21 Supreme Service LLC as the creating and exposing employer did not protect employees from injury in that roofing employees were not given effective information and training on the fall hazard associated with the work being performed as well as how to effectively use their assigned personal fall arrest systems. Employees were working with their lifelines maxed out. Anchors were connected to multiple lifelines. These conditions exposed employees to fall hazards of 8 feet and 10 feet. Abatement Note: Abatement certification and documentation are required for this item. See enclosed "Certification of Corrective Actions Worksheet."

Aurora, CO 80014: On or before 9/23/21 Supreme Service LLC as the creating and exposing employer did not protect employees from injury in that roofing employees were using anchors attached to building facia with more than one lifeline attached to it. Aurora, CO 80014: On or before 9/23/21 Supreme Service LLC as the creating and exposing employer did not protect employees from injury in that roofing employees were using fall protection systems with lifelines that were not rigged to limit free fall. An employee working at the edge of the roof with an 8 foot fall hazard had his lifeline fully extended and would not have prevented a fall.
