29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  On or about September 24, 2021, at 2241 W. Memorial road in Oklahoma City, OK, an employee was allowed to work in the basket of an aerial lift without fall protection attached to the basket or boom, exposing the employee to the hazard of falls.
