29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  On or about September 24, 2021, a medical evaluation was not provide to employee who wore an elastomeric respirator when adding  ingredients, including silica, into a mixer.  Total Wall, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(e)(1) which was contained in OSHA inspection number 1228284, citation number 2, item number 1B, and was affirmed as a final order on September 05, 2017, with respect to a workplace located at390 Viking Circle, Rio WI 53960.

29 CFR  1910.1053(d)(2):The employer did not assess the 8-hour TWA exposure for each employee on the basis of any combination of air monitoring data or objective data sufficient to accurately characterize employee exposures to respirable crystalline silica:  On or about September 24, 2021 employer did not evaluate employees silica exposure when cutting stucco covered foam samples.
