29 CFR 1926.451(f)(6):Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). (a) Jobsite: On or about September 24, 2021, an employee was working from a pump jack scaffold (with aluminum poles) and the pump jack was within 4 feet of the closest overhead uninsulated electric line (2,400 v AC phase to ground and 4,160 v AC phase to phase).

29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Jobsite: On or about September 24, 2021, an employee was working from a pump jack scaffold and he was tied off to a yellow nylon rope that ran from the top of the pump jack poles. The fall hazard was about 40 feet. (b) Jobsite: On or about September 24, 2021, an employee had untied from the yellow nylon rope that ran from the top of the pump jacks poles to exit the scaffolding. The employee traveled approximately 20 feet on an aluminum pic that spanned the one end pump jack pole and the Baker's scaffold and he was not protected by any means from falling about 40 feet to the ground.
