29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On the Phoenix machine, employees engaged in collecting graphite dust samples from the auger trough in the pit on the west side of the machine were exposed to the augers rotating and moving parts. The employer did not ensure a specific hazardous energy control procedure was developed, documented, and utilized.
