1910.134(e)(7)(ii)  Additional medical evaluations that complied with the requirements of 29 CFR 1910.134 were not provided when a PLHCP, supervisor, or the respirator program administrator informed the employer that an employee needed to be reevaluated:(a)     Location: 69 North Plains Highway, Yalesville, CT 06492 - On or about September 24, 2021, the employer did not provide a follow-up medical evaluation when the PLHCP indicated it was required in a year, for a tight-fitting  "MSA 3200" full-facepiece style respirator during surface grinding of concrete building materials.

Location: 69 North Plains Highway, Yalesville, CT 06492 - On or about September 24, 2021, the employer did not ensure that employees required to wear tight-fitting  "MSA 3200" full-facepiece style respirators, while surface grinding of concrete building materials, had received annual fit tests.

29 CFR  1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.   Location: 69 North Plains Highway, Yalesville, CT 06492 - On or about September 24, 2021, the employer did not ensure that employees engaged in surface grinding of concrete building materials, a Table I task, were fully and properly implementing the engineering controls and work practices as specified in Table I.
